The methods in Npinks post obviously work but I would use a socket rather than a spanner, drop a spare nut or two inside the socket so you can push on the bolt with the socket as you turn. also making a fancy bolt like that with washers and extra nuts is ott as in reality you may need more than one per rivnut as it is important to discard any bolts with less than perfect otherwise your rivnut will spin rather than wind down the thread.
