from memory dont think i took it out, just pushed it in about a year ago and never a problem since. before that would play up every day.

thats the right one mate, as you say its loose then that sounds like the problem to me.
Check the wires running into it are ok and if possible open the metal connectors a bit to make a better conne ... |
