Freelander 2 broken cam followers 2014 automatic

I own a Freelander 2 which has broken 2 x cylinder cam followers - after stripping out and replacing all cam followers and exhaust cam shaft , re built and its done the same again on cylinder 1 & 3 after less than 1 mile Sad
any one had this experience

What mileage had the engine when broke ? Did they checked timing belt and aligned ?Problem with injectors. Wrong signals from ECM ? How the was the engine starting and running right before it broke ? Any strange noises ? In both situations ?

Can you clarify a couple of points please, in order to try and understand a bit more of what's happened.

A) Is the second rocker arm breakage the same ones as the first time (same bank exhaust or inlet, same cylinder or cylinders)
B) Did you replace the hydraulic lifters as well as the rocker arms.
C) What caused the original rocker arms to break (timing belt failure? or camshaft lobe failure?)
D) Were genuine replacement LR parts used or pattern parts.

thanks for reply's guys
rocker arms broken same place as before
did not replace hydraulic lifters - will check these
could not find reason for original failure....?
lr parts used for re build.....

Crud blocking an oil way , perhaps the feed to the hydraulic lifters. You would see damage around that area and on the ends of the cam followers.

The rockers aren't known to just break, in fact mine didn't after a nut got in the cylinder (not by me), the valves were destroyed, but the rockers were just fine, not even bent.

The only thing that could cause a rocker to fail will be if the valve it's supposed to be opening isn't moving, either because it's seized to the guide (unlikely unless bent) or the valves are hitting something they shouldn't, like the piston, or a foreign object in the cylinder.

How many rockers on each cylinder are failing?
Maybe the lifters have an issue? They're supposed to move literally 0.5mm, just to take up free play. However if they can't bleed down, then it's possible they could over extend, causing valve contact with a piston. 2009 Rimini Red SE TD4. Soon to be sold.

What work on the engine had been done prior to the breakage, had the cambelt been changed, were the correct engine locking tools used i.e. Starter motor removed to lock flywheel and camshaft locking pin installed. How many miles has the engine covered. Cam followers (rockers) only normally fail when they are unable to de-press the valve down there this woul indicate a there a bent valve or sticking valve.
