Try Settings > safari > autofill and make sure "names and passwords" are turned on.

Whenever I finish browsing I always close all open pages by double clicking the home button and swiping the pages away and then clear the history. Always have done for the last year or so because having an old ipad3 16Gb model that was near to full it seemed to run a lot slower or freeze up if I didn't do that and that always seemed to do the trick but since IOS9, when I cleared the history it would also clear out the cookies for the website logins where it didn't used to before. Apparently the 'trick' is to log in to all your websites before midnight and from the following day when clearing your history, only chose the 'clear today' option and NOT the 'clear all time' option. I've just recently changed to the latest air2 model with 128Gb so I probably shouldn't need to do it anymore anyway. Are those that have been affected also clearing their all time history on shutdown?

Turn it off! Can't remember how as I'm in work on a PC right now but a quick Google will tell you. It's not hard.
